New Player Enrolments & Payments

1. I give permission for my child to participate in all activities of the Bellingen Youth Orchestra in 2022.
2. I give permission:
a. for my child to be photographed or filmed (which may include my child’s voice);
b. for my child’s likeness, picture and biographical information to be published; and
c. for my child’s performance to be edited, reproduced, broadcast or distributed
for the purpose of promoting the Orchestra.
3. If my child is injured or becomes ill during an Orchestra activity I give permission for an authorised person of the Bellingen Youth Orchestra to seek medical treatment for my child from a registered medical practitioner, dentist, hospital or ambulance service and transportation of my child by an ambulance service. An authorised person is limited to the Artistic Director and Orchestra Ensemble Leaders or their delegated Member of the Bellingen Youth Orchestra Association.
4. I consent to the carrying out of appropriate medical, dental or hospital treatment if my child is injured or becomes ill during an Orchestra activity.
5. If medical or dental treatment is required for my child I acknowledge that all costs of such treatment, including hospital treatment and transportation costs by an ambulance service are payable by me and are my responsibility.

1. I have read the BYO Code of Conduct and discussed this with my child.
2. I declare that the information provided on this form is true and correct and understand that it is my responsibility to immediately inform the Bellingen Youth Orchestra Association Incorporated of any changes to the information.
